Job Description: 

Support the transition of youth and young adults to independent living by developingindividualized plans and maintaining connections with family, extended family, and community supports. Identify and contact eligible current and former youth in care to provide information about and enroll them in the Post Majority Care Program. Deliver wrap-around, person-centered supports that promote the holistic well-being, independence, and long-term success of youth. Assist youth in accessing internal and external services, community resources, and cultural programming through appropriate referrals and ongoing support. Contribute to program planning, work plans, and service delivery improvements to enhance outcomes for youth and young adults. Maintain accurate, confidential, and up-to-date client files, records, and documentation in accordance with organizational policies and legislative requirements. Participate in mandatory training, professional development opportunities, cultural events, and other learning activities to support effective service delivery. Provide opportunities for genuine reconnection between youth and young adults to family, culture, and community; provide support that will encourage reunification or repatriation, as required. Assist youth in navigating government, health and social services. Perform additional duties and responsibilities as assigned to support departmental objectives and the overall success of the Post Majority Care Program.
